Details of the MTM Intervention
The Medi-Cal MTM Pilot Program is a Medically Tailored Meals Intervention for Medi-Cal beneficiaries with a diagnosis of congestive heart failure and who are high utilizers of health care services.
The intervention is 12 weeks in duration.

Improve health outcomes and reduce health care costs.
Who is eligible
To be eligible for the pilot program, the Medi-Cal program has identified patients with congestive heart failure who are at high risk of worsening health outcomes.
Twelve months continuous enrollment in Medi-Cal.
A congestive heart failure diagnosis.
Had at least one visit to an emergency department or inpatient visit (hospital or skilled nursing facility) because of a heart failure exacerbation.
Had at least one provider or specialist visit in the last twelve months.
Cost
Free for eligible Medi-Cal patients.
