Health Plan Partnerships

CalFIMC member agencies partner with many health plans and health care providers to implement medically tailored meals (MTMs) and medically tailored groceries (MTGs) and other nutrition services. As experienced Community Based organizations in nutrition services, the CalFIMC agencies offer services to thousands of vulnerable Californians.

They are contracting with health plans across the state to provide MTMs as part of Community Supports in CalAIM (California Advancing and Innovating Medi-Cal). They also undertake pilot programs with health plans and healthcare providers.

Please see the CalFIMC member agency section of this website to find out how to reach the local agency in your area.

Below are examples of some of the health plan partners for these programs.

Health Plan Partner Testimonials

“Anthem Blue Cross understands that addressing social drivers, such as access to healthy food and nutrition programs, is the key to improving health outcomes. That is why we are committed to investing in new programs and partnerships that expand access to both healthcare and the life essentials that form the foundation of healthy living.”

–Marsam Kasravi, MD, President, Anthem Blue Cross

We’re very anxious to see a program like this lead to policy changes. Particularly in Medi-Cal, where dollars are precious, if we can demonstrate that providing meals to people with congestive heart failure reduces their readmission rates in the hospitals, that would be a great step forward.

–John Baackes, CEO, L.A. Care Health Plan
