Health Plan Partnerships
CalFIMC member agencies partner with many health plans and health care providers to implement medically tailored meals (MTMs) and medically tailored groceries (MTGs) and other nutrition services. As experienced Community Based organizations in nutrition services, the CalFIMC agencies offer services to thousands of vulnerable Californians.
They are contracting with health plans across the state to provide MTMs as part of Community Supports in CalAIM (California Advancing and Innovating Medi-Cal). They also undertake pilot programs with health plans and healthcare providers.

Below are examples of some of the health plan partners for these programs.
Anthem Blue Cross: Partnership with Project Open Hand, Project Angel Food, and The Health Trust, in San Francisco, Alameda, Los Angeles, and Santa Clara counties; supporting a MTM pilot for clients with Congestive Heart Failure, End-Stage Renal Disease, and Gestational diabetes.
Blue Shield Promise: Partnership with Mama’s Kitchen in San Diego County; supporting a MTM pilot for clients with congestive heart failure and other illnesses
Central California Alliance for Health: Partnership with Teen Kitchen Project in Santa Cruz County; supporting a MTM program for Medi-Cal members with diabetes, CHF, and/or COPD at high risk for hospital readmission.
Kaiser Permanente: Partnership with Project Open Hand and Ceres Community Project in Marin, Sonoma and Santa Clara counties, testing Medically Tailored Meals (MTM) or MTM + Medical Nutrition Therapy (MNT) for clients with Chronic Kidney Disease, Congestive Heart Failure, diabetes and/or COVID-19 at discharge from the hospital.
L.A. Care Health Plan: Partnership with Project Angel Food in L.A. County to support three medically tailored meal programs for clients with diabetes, Chronic Heart Failure, Chronic Kidney Disease, and people impacted by COVID-19.
San Francisco Health Plan: Partnership with Project Open Hand in San Francisco County; a MTM pilot to support clients with CHF and other illnesses, including people affected by COVID-19
